NEW YORK DOMESTIC RELATIONS LAW (Chapter 14) § 21 — Forms and books to be furnished DRL § 21 Forms and books to be furnished /legislation/laws/DOM/A3 § 21. Forms and books to be furnished. The proper books forregistration, blank forms for marriage licenses, certificates,statements and affidavits and such other blanks as shall be necessary tocomply with the provisions of this article, shall be prepared by thestate department of health and shall be furnished by said department atthe expense of the state to the town and city clerks filing records withthe state department of health in such quantities as their necessitiesshall require.
